Configurable MIDI clock smoothing, add bandwidth to midi clock params. Add pulse out to midi clock input

Chaos controller external input option added.

ADSR times can be longer.

MIDI learn was responding on all channels after learn. Fixed so only responds to channel CC was sent on. You will need to re-learn saved CCs in your patches.

Envelope follower MIDI learn was scaling incorrectly.

Fix to start up some times failing on some power supplies. This does make start up slower for some people as it waits for the supply voltage to stabilise.

Fix channel 1 and 2 being out of phase on some units.
